All healthcare practitioners applying for initial licensure or renewing an existing license must complete a background screening via fingerprinting. This includes:

Get Your TCN (Transaction Control Number)
This is your tracking number—don’t leave the appointment without it.
Use it to monitor your submission status via FDLE’s TCN Lookup tool.
Processing time:
- 24–72 hours depending on FDLE/FBI turnaround
- Fingerprint retention: Required every 5 years for most professions

Specifically, the following types of providers and suppliers are generally considered high-risk and subject to the fingerprinting requirement:
Skilled Nursing Facilities (SNFs): Including those newly enrolling, revalidating, or adding new locations.
Durable Medical Equipment, Prosthetics, Orthotics (DMEPOS) Suppliers: Including prospective (newly enrolling) suppliers.
Home Health Agencies (HHAs): Including those newly enrolling.
Medicare Diabetes Prevention Program (MDPP) suppliers:
Opioid Treatment Program (OTP) providers.
CMS sends notification letters to affected providers, outlining the fingerprinting requirement and providing instructions for the process, including the designated contractor for fingerprint collection and submission. Providers have a specific timeframe, typically 30 days, to comply with the notification. Failure to comply can result in denial of enrollment or revocation of billing privileges.

If you're living outside the U.S. but need fingerprinting for licensing or employment in Florida, there's no need to travel! ACT Biometrics makes the process simple and hassle-free by converting your fingerprint card into Live Scanand submitting it directly to the Florida Department of Law Enforcement (FDLE). FDLE then forwards the results to the requesting agency, such as the Department of Health (DOH), AHCA, DCF, APD, and others.
Follow these easy steps to complete your fingerprint submission:
Step 1: Pay for the Service
Book an appointment online to pay for the fingerprinting service.
Note: This booking is only for payment—applicants do not visit the office for fingerprinting.
Step 2: Get Fingerprinted
Visit your nearest law enforcement office or fingerprint provider to have your fingerprints taken.
Ensure the fingerprints are recorded on an FD-258 fingerprint card, which can be downloaded from the FBI website or requested from ACT Biometrics.
Step 3: Submit Your Fingerprint Cards
Mail the completed fingerprint cards to: ACT Biometrics 8469 W. Oakland Park Blvd, Sunrise, FL 33351
If applying for a healthcare license or employment, you must also provide:
Social Security Number (SSN), Taxpayer Identification Number (TIN), or Canadian SIN
A passport-style photo (submitted via email)
Step 4: Receive Your TCN Number
Once ACT Biometrics processes your fingerprints, you will receive a Transaction Control Number (TCN) upon submission of the livescan.
